目的 观察小牛血清去蛋白注射液对心房纤颤(房颤)所致脑栓塞患者神经功能缺损和日常生活能力的影响及安全性.方法 2016年1月~2017年1月于新疆医科大学第六附属医院就诊的房颤所致的脑栓塞73例患者进行了研究,73例患者随机分为治疗组38例和对照组35例,两组静脉滴注阿加曲班(10~30 mg/d),依达拉奉(30 mg,2/d),静滴甘露醇(125 ml,1/6 h);治疗组在对照组基础上加用小牛血清去蛋白注射液(20 ml,1/d).14 d为一疗程.分别采用美国国立卫生院卒中量表(NIHSS)、Barthel指数(BI)评价临床神经功能缺损和日常生活能力,以NIHSS为主要参考指标比较两组临床疗效,记录肝肾功能、血尿常规及心电图等不良反应.结果 治疗组中平均年龄(67.36±8.64)岁,男性20例,病程(3.91±2.03)d.对照组患者的平均年龄(66.83±8.82)岁,男性17例,病程(4.13±2.37)d.两组一般资料比较差异无统计学意义(P>0.05),治疗后,与对照组相比,治疗组NIHSS明显降低,BI明显升高;治疗组临床有效率为85.46%,高于对照组的64.88%(P<0.05).两组未见明显不良反应.结论 小牛血清去蛋白注射液可以明显改善房颤所致脑栓塞的患者的神经功能缺损和日常生活能力.
Objective To observe the impacts of deproteinized calf blood serum injection on the neurological function impairment and activity of daily living of patients with cerebral embolism caused by atrial fibrillation, and the safety. Methods 73 cases were randomly divided into study group (38 cases) and control group (35 cases), they were both treated by Argatroban (10~30 mg/d), Edaravone (30 mg each time, once a day) and Mannitol (125 ml each time, 1/6 h), intravenous drip. The study group received deproteinized calf blood serum injection (20 ml each time, once a day) additionally. All treated for 14 days. National Institute of Health Stroke Scale (NIHSS) score and Barthel index (BI) were calculated to assess the neurological function impairment and activity of daily living, respectively. Taking NIHSS as the main reference index, the clinical efficacy of the two groups was compared, and the adverse effects of liver and kidney function, blood and urine routine and electrocardiogram were recorded. Results There was no significant difference in the general data between the two groups (P>0.05). After treatment, as compared with those in the control group, NIHSS score was significantly reduced and BI score was increased in the study group (P<0.05). The clinical efficiency was 85.46% in the study group, which was higher than 64.88% of control group. No obvious adverse reaction was recorded. Conclusion Deproteinized calf blood serum injection could improve the neurological function impairment and activity of daily living of patients with cerebral embolism caused by atrial fibrillation.
